Who is the controller

The controller of the data given in an order form is the operator of the Formulateka service. For matters concerning personal data, write to kontakt@formulateka.com.

These pages carry no form

The English part of the service is the choice of a country and the description of our method. There is no order form on it, so no name and no telephone number are collected here. Order forms stand in the country sections, next to a product, and the path of their data is described by the policy of that section, the one written in the language of that country.

• Poland: the section with the forms and its own policy.

What an order form collects

Where a form exists, it collects a first name and a telephone number. In addition the IP address and the address of the page the form was sent from are processed: the distributor needs them to attribute the order and to verify it. The data goes to the distributor of the preparation the order concerns, and it is that party who calls back and ships the parcel. We pass the data to nobody else.

How long it is kept

The service keeps no order database of its own: the data is passed to the distributor, and the period of storage is on that side, under the rules in force there. Who is party to the sale is written in the Terms of use; who runs the service, in About us.

Your rights

• access to your data and its correction;
• erasure of the data;
• restriction of processing;
• objection to processing;
• a complaint to the supervisory authority of your country.

Cookies and browser storage

The service sets no cookies of its own for tracking and embeds no advertising tools of third parties.

In the local storage of the browser (localStorage) one entry is written, named formulateka_popup_seen. It remembers that the window with the order form has already opened once by itself, so that it does not open again. The entry appears on product pages, stays on your device, never reaches our server and identifies nobody. Clearing the site data in the browser settings removes it.
